| Jay I've seen that kind of thing happen to nearly-new tires due to potholes or road debris. Anything that causes the belts to separate will end up like that. |

We had it happen on a few new tires, but they were the cheap ass ones. I mostly saw it on old ones.
But none the less, it causes vibrations |

the bolt that i had going through the mount was a little short so i could see how the nut could have fallen off, but i replaced it with a longer one _________________

| You need to get the correct graded bolts and locking nuts in there or it will continue to be an issue. |
